Membership Terms, Billing & Cancelation Policy

The Nit Nurse Lice and Nit membership includes 12 free lice screenings per membership. One screening per calendar month. Screenings do not rollover to the following month.
Free treatments. Two per membership year are included in the membership.
Covered membership appointments must be scheduled during normal business hours. Between 9am and 5pm, Monday through Friday. Visits afterhours or on weekends will be subject to after-hours charges, not covered in the membership.
Membership must be active at least 30 days prior to the ability to utilize benefits.
Member must be currently enrolled in the plan and current on payment plan to utilize benefits.
Member’s credit card will be automatically billed $19.95 monthly if monthly plan is selected.
Member’s card will be billed once for the discounted annual rate of $215.00 if the annual purchase option is selected.
This membership will not auto renew once the year membership has completed.
Each member of the household, as well as all members of the secondary house hold (ex: split homes) must be screened and cleared of lice or treated by The Nit Nurse if and when a plan member is found to have an infestation. All applicable fee’s for head checks and treatments will apply with a 10% discounted rate for family members not covered by The Nit Nurse Lice and Nit Protection Plan.
Minimum plan commitment of 12 months.
Early termination, either by member choice or due to non-payment, will be subject to a $100.00 cancelation fee, as well as any collection fees. All unpaid accounts will be referred to a collection agency.
I understand and authorize The Nit Nurse to charge any outstanding fees such as no show fee, collection fee, and cancelation fees to the card on file without prior authorization from card holder.
The Nit Nurse reserves the right to deny or terminate any membership without notice at owner’s discretion.
I understand that if an appointment is schedule and I miss the appointment or do not provide The Nit Nurse at least 24 hours’ notice I will be subject to a $50.00 cancelation/no show fee. This fee will be assessed per person. This fee will occur no matter if client is a membership member or not. I authorize this charge to be completed without prior authorization.
I agree to a $20.00 dollar fee for failed payments. This fee is to be completed without prior authorization.
After 31 days delinquency, The Nit Nurse will terminate the plan membership and the member will be billed a $100.00 early termination/cancellation fee per member.
The Nit Nurse reserves the right to revise its terms and conditions.

However, if you choose to participate in any of our online activities, you will be asked for applicable personal data, such as your name, email address, phone number, etc. The purpose of collecting such data is to better respond to specific questions and requests for help or to facilitate support
Sharing Your Information with Third Parties
- The Nit Nurse never shares your e-mail addresses with third parties without your consent. Personal information collected is not rented or exchanged to any third parties.
